National Background Check

 

National Background Check Program for Adults Volunteering with MYSA and MUSC

MYSA's new process will allow each adult to go online to a secure website and fill out the necessary information. MYSA is excited about this new, expanded program and very proud to be one of the few state associations taking the lead on providing its youth and adults with such a comprehensive program aimed at protecting many of those involved in youth soccer in Minnesota.

1. Open the easy-to-follow instructions (pdf)

2. Open the TRUE Background Screening Sign-In website

MUSC username and password
Username: MYSMNK (must type in CAPS)
Password: MYSMNK (must type in CAPS)

--- OR FOLLOW THESE TEXT-BASED INSTRUCTIONS ---

1. Go to www.mcdowellagency.com and click on the “Client Log-in” tab   found at the top right.

2. Enter user id: MYSMNK and password: MYSMNK and click on “Login.” [note the id and password are ALL UPPERCASE]

Click on the “Request” tab found at the top of the webpage.

3. Type full name into the “Electronic Signature” box and mark the box next to “My name entered above signifies my Electronic Signature” and click on “Continue” to proceed.

4. The following information is required to complete the background check:  last name, first name, current address, city, state, zip code, date of birth, social security number and gender; then click on “Continue” to proceed.

5. Under “Criminal Court Searches” – click on “Add/Edit Jurisdictions.”

5a. Under “Search Type” click on drop down box and click on “Felony and Misdemeanor,” under “State” click on drop down box and click on “MN,” under “County/Jurisdiction” click on county of residence (in alphabetical order).

5b. Under “Search Type” click on drop down box and click on “Felony and Misdemeanor,” under “State” click on drop down box and click on “MN,” under “County/Jurisdiction” click on “Statewide” (located at the bottom of the selection list).

5c. Click on “Add Searches to Order.”

6. Social Trace, National Sex Offender Database and USA Criminal Index are prefilled.  Click on “Agree and Submit Order.”

7. Print receipt and log off.
